Abstract
Telemedicine is transforming pharmacy practice, especially in the management of chronic conditions, by offering an efficient and accessible way to deliver pharmaceutical care. Remote consultations with pharmacists, medication therapy management, and virtual follow-ups have improved medication adherence, better disease control, and enhanced patient satisfaction, making telemedicine a crucial tool for patients requiring ongoing pharmaceutical support and monitoring. However, challenges such as limited access to technology, concerns over data privacy, and the need for adequate training for both patients and pharmacy professionals must be addressed for telemedicine to reach its full potential in the pharmacy sector.This review examines the evidence supporting the effectiveness of telemedicine in chronic disease management, particularly in the context of pharmacy services. It highlights the benefits of telepharmacy in improving patient outcomes, reducing hospitalizations, and enhancing medication management through virtual consultations and remote monitoring. The review also identifies barriers to the broader adoption of telemedicine in pharmacy, such as technological limitations, regulatory issues, and unequal access to digital resources.
